Job Description:

We are seeking a skilled Social Worker to join our team at Tri-County Mennonite Homes, providing exceptional care and support to residents in our long-term care homes.

About the Role:

This is an excellent opportunity for a self-directed and energetic individual to utilize their expertise in social work to make a meaningful difference in the lives of our residents.

The successful candidate will work collaboratively with residents, caregivers, and other team members to provide person-centred care and support spiritual needs.

A key aspect of this role is overseeing Spiritual Care needs for residents, planning liturgies and worship services, and coordinating pastoral care at the end of life.

Key Responsibilities:
  • Assess and address the spiritual needs of residents
  • Develop and implement care plans that prioritize resident well-being
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Provide education and support on grief and loss to families and staff
Requirements:

To be successful in this role, you will have:

  • A relevant degree or diploma in social work (SSW, BSW, MSW)
  • Current registration with the Canadian Association of Social Workers and/or Ontario College of Social workers
  • Previous experience in a long-term care home or healthcare setting
  • Strong interpersonal, communication, and organizational skills
What We Offer:

We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience, plus benefits, including pension plan, health and dental insurance, short-term disability, and life insurance.
