Comprehensive Social Work Role at Northern Health Authority

We are seeking a skilled and compassionate healthcare professional to fill a comprehensive social work role at our facility in Quesnel, BC. This position offers an excellent opportunity to make a meaningful difference in the lives of patients and families in our community.

About the Role:
  • This is a full-time position with regular working hours (07:00 to 18:12) and rotating shifts.
  • You will be part of our interdisciplinary team, working collaboratively with other healthcare providers and stakeholders to provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
  • Your expertise in psychosocial care, education, navigation, and advocacy will enable you to maintain or improve the health and social well-being of patients/clients/residents and their families.
Key Responsibilities:
  • Provide direct psychosocial care, education, navigation, and advocacy to patients and families.
  • Collaborate with the multidisciplinary team to develop and implement comprehensive care plans.
  • Maintain accurate and up-to-date records of patient interactions and progress.
  • Develop and maintain relationships with community agencies and organizations to ensure seamless transitions of care.
Requirements:
  • Bachelor's Degree in Social Work from an accredited university.
  • Two (2) years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders.
  • Current registration with the British Columbia College of Social Workers.
  • Current valid BC driver's license.
Skills and Qualifications:
  • Demonstrated ability to communicate effectively, both verbally and in writing.
  • Demonstrated ability to provide service in a professional manner consistent with current social work practice standards.
  • Demonstrated ability to engage in work with a variety of acutely behaviorally, psychologically, emotionally distressed patients and families.
  • Demonstrated ability to adapt and be flexible to changing demands and environments.

The estimated annual salary for this position is $65,000 - $85,000, depending on qualifications and experience.
