At Qualicare North York, we are seeking a highly skilled Registered Practical Nurse (RPN) to fill the position of Care Manager. This role is responsible for planning, directing, and evaluating client and staff care, as well as implementing the standards of care.

Key Responsibilities:

  • Implement administrative and client care policies and practices for the Qualicare location in compliance with provincial regulations.
  • Consult with supervisor and staff, individually or in groups, regarding clients, families, special programs, and service programs.
  • Oversee client care management processes, including intake-to-client conversions, case set-up, customer service, and converting inquiries to service provision.
  • Assure that skilled nursing and other therapeutic services are provided under the supervision and direction of a physician or a Registered Nurse.
  • Consult with physicians in matters relating to client care services.
  • Direct the implementation of service goals and objectives.
  • Develop standards that ensure safe and effective services to clients and families.
  • Oversee the implementation and evaluation of client care services, inclusive of regulatory licensure, certification criteria, and accreditation standards.
  • Collaborate in the development of an effective and efficient client care documentation system, including statistical compilation and analysis relative to cost and staff service delivery.
  • Achieve/exceed client satisfaction and investigate issues and follow up promptly.

Requirements:

  • Registered Practical Nurse (RPN) with a minimum of five (5) years of clinical experience, with at least half of that time working in home health or a related healthcare organization.
  • Minimum three to five years of direct employee supervision.
  • Exceptional nursing assessment skills.
  • Proven ability to build quality and implement improvements with the operation.
  • Excellent interpersonal skills and analytic/critical thinker.
  • Excellent customer service skills and ability to deal tactfully with clients, staff, and community.
