Job Summary: We are seeking a skilled Licensed Practical Nurse to join our team as a Wound Care Specialist in Home Health. As a member of our interprofessional healthcare team, you will work collaboratively to deliver high-quality patient care and promote optimal health outcomes.

Key Responsibilities:

  • Assess, plan, implement, and evaluate the needs of clients and their caregivers/families in a variety of settings.
  • Collaborate with Registered Nurses and other healthcare professionals to provide direct nursing care to individuals across the lifespan.
  • Develop and implement wound care plans, including skin and wound management, to promote optimal healing and prevent complications.
  • Provide education and support to clients and their caregivers on wound care and management techniques.
  • Participate in interprofessional healthcare teams to coordinate care and ensure seamless transitions between settings.

Requirements:

  • Completion of an accredited practical nurse education program.
  • Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
  • Minimum 3 years of experience in Integrated Home Care, with a focus on wound care management.
  • Ability to work independently and as part of an interprofessional team.
