Mental Health and Addictions Support Worker Role
At PHSA, we are seeking a skilled Mental Health and Addictions Support Worker to join our team. As a key member of our team, you will provide essential support to individuals with complex mental health and substance use challenges.
Key Responsibilities:
- Monitor clients and document observations in accordance with standard procedures.
- Participate in the admission, case planning, and discharge process for clients.
- Establish rapport with clients and provide emotional support and counseling.
- Assist clients with daily living activities and escort them to medical and social appointments.
- Participate in case planning and coordinate services with medical professionals.
Requirements:
- Diploma from a recognized Social Services Support Worker educational program.
- Two years' recent, related experience, or an equivalent combination of education, training, and experience.
- Valid driver's license.
