Home Infusion Nurse

Job Summary
We are seeking an experienced Home Infusion Nurse to join our team of Specialty Pharmacists. As a Home Infusion Nurse, you will be responsible for delivering and monitoring specialized infusion, injectable or oral pharmacological treatments in the home and/or clinic setting.

Responsibilities
- Develop and review individualized nursing care plans for patients receiving infusions at home or in clinic settings.
- Conduct patient health assessments and medication administration teaching to ensure optimal patient outcomes.
- Obtain IV access through peripheral IV initiation or central line access (PICC, IVAD, etc.).
- Prepare and administer a variety of specialized intravenous or injectable pharmacological treatments as prescribed by physicians.
- Provide patient-centered care according to established care plans or medical directives, ensuring seamless coordination with healthcare teams.
- Maintain accurate records of patient interactions, treatment plans, and outcomes to ensure compliance with regulatory requirements and Bayshore's standards.
Requirements
- Bachelor's degree in Nursing (BSN) from an accredited nursing education program.
- Current registration and good standing with provincial nursing regulatory body.
- Basic Cardiac Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) certification required for clinic role.
- Chemotherapy – Biotherapy course completion is an asset.
- Minimum 2 years of experience in acute care and/or administering specialty pharmaceuticals.
- Critical care, infusion clinic, or oncology experience is preferred.
