Case Manager

We are seeking a detail-oriented Case Manager to join our team at AMHS-KFLA. In this role, you will work closely with clients to assess their needs and develop personalized plans to achieve their goals.
Key Responsibilities:- Conduct intake assessments to identify client needs and goals
- Develop and implement case plans to meet client needs
- Collaborate with internal and external stakeholders to provide comprehensive support to clients
- Monitor and evaluate client progress and make adjustments to case plans as needed
- Bachelor's degree in a related field
- 2 years to less than 3 years of experience in case management or a related field
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
