Community Rehabilitation Specialist

Mountains and Meadows Care Group is seeking a Community Rehabilitation Specialist to join our team.
About the RoleThis is a rewarding opportunity to make a difference in the lives of individuals in our community. As a Community Rehabilitation Specialist, you will work with clients living in group homes and half-way houses, providing support and guidance to help them achieve their goals.
Key Responsibilities- Assess client needs and eligibility for services
- Develop and implement service delivery plans
- Provide administrative support and perform various tasks as required
- Supervise activities and provide guidance to clients
- Accompany clients to appointments and social outings
To be successful in this role, you will require:
- A certificate or diploma in a relevant field (e.g. Behaviour Management Certification, Community Disability Studies Certificate)
- Experience working with clients with varying abilities
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
We offer a competitive salary range of $55,000 - $65,000 per year, depending on experience.
