Regional Palliative Care Consultation Team Lead

We are seeking a motivated Registered Nurse with a dedication for delivering compassionate patient care to join our Elder Programs, Hospice and Palliative care department. The Regional Palliative Care Consultation Team Lead has a strategic and operational role to support and champion an interdisciplinary team comprised of physicians and nurses from each HSDA and a regional pharmacist. The Lead is responsible to support the consultative model, which is foundational to the NH HPC Program. The operational responsibilities include the ongoing development, performance monitoring and evaluation of the NH HPC Consultation Team to promote excellence in hospice palliative care. The strategic responsibilities involve ensuring the alignment and coordination of all aspects of HPC initiatives and existing HPC services to support the vision, goals and objectives, policies, and procedures of the NH HPC Program. The Lead will establish a strong collaborative relationship with Home & Community Care, Northern Cancer Control Strategy, acute care sector, all HSDA managers, HPC advisory groups per HSDA, and other partners.

Requirements
Registration with BC College of Nurses and Midwives as a practicing RN registrant.
Two years recent palliative care experience.
Five (5) years recent related clinical practice experience within the last seven (7) years, where relevant nursing skill and management principals have been utilized, including two years successful formal supervisory experience in a health care setting.
Vehicle, valid driver's license required and appropriate insurance.
