Nurse Specialist in Palliative Care

Job Description:
The Registered Nurse will play a key role in providing exceptional end-of-life care to residents at Stedman Community Hospice. As part of the interdisciplinary team, you will contribute to the development, implementation, and communication of individualized care plans and ensure seamless transitions across the continuum of care.
Responsibilities:
• Provide leadership and guidance to the care team in implementing evidence-based best practices in pain and symptom management, as well as end-of-life care;
• Develop and maintain effective relationships with patients, families, and healthcare providers to promote a patient-focused approach to care;
• Collaborate with the healthcare team to assess, plan, implement, and evaluate care, ensuring that all aspects of resident care are met;
• Recognize and respond to resident health status changes, advocating for timely interventions to prevent unnecessary complications;
• Participate in ongoing education and training opportunities to enhance knowledge and skills in palliative care;
Requirements:
• Current registration with the College of Nurses of Ontario (CNO);
• Minimum 2 years of experience in nursing, with a strong background in hospice and palliative care;
• Demonstrated expertise in pain and symptom management, as well as end-of-life care;
• Strong clinical practices and ability to champion evidence-based best practices;
• Excellent problem-solving skills and ability to manage complex clinical situations;
• Graduate of an approved Registered Nursing program and current CPR and First Aid certification;
