Care Coordinator

We are seeking a highly organized and detail-oriented individual to join our team as a Care Coordinator at Hawthorne Place Care Centre Facility Inc.
Key Responsibilities- Coordinate the flow of information and maintain accurate records
- Manage incoming mail and other materials, and maintain a filing system
- Order supplies and maintain inventory
- Enter and format medical reports and correspondence, and prepare spreadsheets and documents for review
- Initiate and maintain confidential medical files and records
- Perform data entry and provide customer service
- Schedule staff and resolve conflict situations
- Perform reception and clerical duties
- Employ creative problem solving to identify opportunities for process improvement
- Secondary (high) school graduation certificate
- 3 years to less than 5 years of experience
- Ability to work in a nursing home/home for the aged environment
- Excellent communication and organizational skills
- Health care plan
