Care Coordinator Role

The Care Coordinator is a vital member of our team, responsible for building strong relationships with patients, staff, funders, and referral sources. This is achieved by providing administrative support and coordinating clinic activities with excellent customer service.

Key Responsibilities:

  • Answer telephone calls, greet patients, and provide exceptional customer service.
  • Coordinate patient intake, data entry, and scheduling of appointments.
  • Assist patients with paperwork, collect payments, and track coverage for funding streams.
  • Complete data entry, billing, and coordinate courier packages.
  • Provide administrative support to the Care Coordinator Manager and Clinic Director.

Qualifications & Core Competencies

  • High School Diploma, G.E.D., or equivalent.
  • Excellent customer service skills, telephone etiquette, and organizational abilities.
  • Strong communication, interpersonal, and computer skills.
  • Flexible and adaptable to change.
