Job Summary

Ottawa Inner City Health is seeking a highly skilled and compassionate Nurse Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our clients who are experiencing homelessness.

Key Responsibilities
  • Clinical Care
    • Provide comprehensive assessments of clients' needs, including health, housing, and social services requirements.
    • Develop and implement individualized care plans in collaboration with clients and other healthcare professionals.
    • Coordinate care activities and ensure effective communication between team members.
    • Monitor clients' health status and take appropriate action to meet changing needs.
  • Leadership and Collaboration
    • Provide leadership to an interdisciplinary team of healthcare and social services providers.
    • Collaborate with shelter and housing staff to ensure high-quality care for our clients.
    • Supervise and support Client Care Workers and volunteers.
  • Advocacy and Education
    • Act as an advocate for clients and the program, ensuring access to necessary services and resources.
    • Provide learning experiences for healthcare students and participate in research and evaluation activities.
  • Administrative and Organizational
    • Utilize electronic medical records systems to document client information.
    • Collect and report data to the City of Ottawa Public Health Department.
    • Ensure adherence to policies, procedures, and practices of Ottawa Inner City Health and partner agencies.
Requirements
  • Current license as a Registered Nurse in the province of Ontario.
  • 2-5 years' experience working with homeless or vulnerable populations.
  • Good knowledge of mental health and addictions, with demonstrated competence in clinical nursing practice related to adult homeless persons.
  • Strong organizational and communication skills, with ability to teach, consult, and negotiate.
  • Ability to work independently and as part of a team, with a commitment to harm reduction strategies.
  • Familiarity with computer software, including EMR, email, word documents, and spreadsheets.
  • Up-to-date vaccinations and current CPR/AED certification.
