Salary: $85,000 - $110,000 per year
Job Description
We are seeking an experienced Geriatric Emergency Nurse Specialist to join our interdisciplinary team at Providence Health Care. The successful candidate will possess expertise in assessing and managing complex geriatric care needs, with a strong focus on promoting healthy behaviors and empowering patients to take control of their health.
Duties and Responsibilities:
- Assess patients' physical, emotional, and mental readiness for learning, through methods such as physical assessments, discussions, completion of questionnaires, chart review, and collaboration with other team members.
- Develop educational materials for patients and families, including pamphlets, brochures, instruction sheets, and manuals.
- Provide direct nursing care, including administering medications and treatments.
- Consult with the Geriatric Care Team and ED interdisciplinary healthcare teams to provide education and direction in managing acutely ill seniors.
- Act as an education resource for staff, teaching and educating members of the interdisciplinary team as required.
- Collaborate with ED staff to identify patients who are appropriate for admission and facilitate transfer to in-patient units.
- Make recommendations for discharge plans and follow-up services in collaboration with the interdisciplinary team.
- Follow up as necessary to ensure continuity of care.
Required Skills and Qualifications:
- Registration with the British Columbia College of Nurses and Midwives as a practicing RN registrant.
- Three years' recent related clinical experience in emergency or gerontological nursing.
- Recent acute care experience and one year's leadership experience.
- Familiarity with relevant Home Health programs.
- Completion of post-graduate specialty program(s) and/or Canadian Certificate in Gerontology.
