Community Support Worker

Pacific Coast Community Resources is seeking a dedicated and compassionate Community Support Worker to join our team. As a Community Support Worker, you will play a vital role in providing care and support to individuals living in staffed homes within their community.

Your primary responsibility will be to empower and support individuals to live independently and self-determiningly, while maintaining excellent standards of care, health, and safety. You will work collaboratively with a multidisciplinary team to ensure the supported persons' rights and choices are respected and advocated for as needed.

Key responsibilities include:

  • Encouraging and empowering individuals to be independent and self-determining
  • Providing care and support to individuals living in staffed homes
  • Developing and implementing Personal Service Plans
  • Fostering positive relationships with individuals, families, and community members
  • Recording daily activity and personal health information
  • Maintaining a clean and well-maintained home environment
