Job Title: Rehabilitation Support Worker

Job Summary: We are seeking a highly motivated and detail-oriented Rehabilitation Support Worker to join our team at St. Paul Abilities Network (Society). As a key member of our rehabilitation team, you will provide support and services to individuals with disabilities, promoting independence and community integration.

Responsibilities:

  • Provide individualized support and services to clients with disabilities, including assistance with daily living activities, personal care, and community access.
  • Develop and implement personalized rehabilitation plans, in collaboration with clients, families, and healthcare professionals.
  • Conduct assessments and evaluations to identify client needs and goals, and develop strategies to achieve them.
  • Collaborate with other healthcare professionals, community agencies, and stakeholders to ensure comprehensive and coordinated care.
  • Provide education and training to clients, families, and caregivers on rehabilitation techniques, strategies, and resources.

Requirements:

  • Education: College, CEGEP, or other non-university certificate or diploma from a program of 3 months to less than 1 year.
  • Experience: 7 months to less than 1 year.
  • Criminal record check, Child Abuse Registry check, Driver's validity licence check, Adult Abuse Registry check.
  • Ability to work independently, work under pressure, physically demanding, attention to detail, large workload.
  • Accurate, efficient interpersonal skills, excellent oral communication, interpersonal awareness, judgement, values and ethics, client focus, excellent written communication, flexibility, initiative, team player, organized.
