Job Summary

We are seeking an experienced Nurse Practitioner to join our team at Can-NP-Serv, a leading provider of clinical services in Ontario. This is a full-time, permanent position that offers a competitive salary and excellent benefits.

About the Role

The successful candidate will be responsible for delivering high-quality medical care to long-term care residents, managing stable complex chronic co-morbidities in collaboration with physicians, and participating in interdisciplinary team meetings to ensure comprehensive patient care. Additionally, this role will involve providing educational support to clients and staff, as well as contributing to quality improvement initiatives.

Key Responsibilities
  • Responsible for the delivery of medical care to long-term care residents, including assessment, diagnosis, and treatment planning.
  • Collaborate with physicians and other healthcare professionals to manage complex chronic conditions.
  • Participate in interdisciplinary team meetings to ensure comprehensive patient care.
  • Provide educational support to clients and staff on various health topics.
  • Contribute to quality improvement initiatives to enhance patient outcomes.
Requirements

To be considered for this role, you must have:

  • A Bachelor's degree in Nursing and a Master's degree in Nursing or a related field.
  • A minimum of 1 year of experience as a Nurse Practitioner, preferably in geriatric care.
  • Certification with the College of Nurses of Ontario.
  • Excellent communication and interpersonal skills.
  • A commitment to providing high-quality patient care.

Our flexible schedule allows for 8-hour shifts from Monday to Friday.
