Job Summary

Halton Healthcare is seeking a skilled Social Worker to join our interdisciplinary Step-Up Neuro Rehab team. As a key member of our team, you will play a vital role in assessing, planning, implementing, and evaluating patient needs, as well as working with patients' families and/or significant others.

Key Responsibilities
  • Comprehensive Assessments: Conduct thorough social work assessments using various modalities and in accordance with professional standards and clinical practices and policies.
  • Resource Connection: Consider individual needs and readiness of patients/families/caregivers when providing resources and connecting them to community services.
  • Patient Education: Provide education to stroke/neuro patients and their families regarding what to expect as they transition back into the community.
  • Interdisciplinary Collaboration: Collaborate with the inter-disciplinary team to address ongoing patient needs, participate in discussions to help optimize patient care, and ensure the coordination of community-based support services.
  • Socio-Emotional Support: Engage patients to assist with adjustments related to socio-emotional health issues and identify patients' networks of supports.
  • Care Planning: Formulate a care plan to accomplish patients' goals.
  • Conflict Resolution: Resolve complex issues with patients/families through the use of conflict resolution, providing mediation, and advocacy both with internal and external partners.
Qualifications
  • Education: BSW or MSW required.
  • Registration: Current registration and membership in good standing with the College of Social Workers and Social Service Workers.
  • Experience: Minimum 3 years of experience in a hospital or community health organization.
  • Specialization: Experience working in neurology/stroke settings preferred.
  • Skills: Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interprofessional team members, and community stakeholders through face-to-face or telephone interventions, class teaching, group facilitation, and adult learning.
  • Implementation: Skillful implementation of planned social work interventions, both individually and in group settings, with the patient, family, caregiver, inter-disciplinary team, and community resources.
  • Canadian Stroke Best Practice Guidelines: Able to address the unique health, social, and service challenges for the patient/family/caregiver in keeping with Canadian Stroke Best Practice Guidelines.
  • Initiative and Multitasking: Demonstrates a high degree of initiative, self-direction, and ability to multitask in a fast-paced environment.
  • Teamwork: Demonstrated ability to function effectively as a member of the healthcare team.
  • Employment Record: Satisfactory employment and attendance record.
  • Community Knowledge: Comprehensive knowledge of community resources and research skills to secure new resources.
  • Patient-Centered Care: Cares passionately about the needs and well-being of patients, families, and caregivers and the needs of our local community.
