Salary Information

The salary range for this position is approximately CAD $42 per hour to CAD $56 per hour, depending on experience and qualifications.



Job Description

As a Discharge Follow Up Nurse with Vancouver Coastal Health, you will play a critical role in ensuring seamless transitions from acute care to community settings. You will work closely with healthcare teams to monitor patients' progress, provide education and support, and coordinate follow-up care.


Your responsibilities will include:



  • Conducting comprehensive assessments to identify patients' needs and develop personalized care plans.
  • Collaborating with interdisciplinary teams to ensure timely and effective communication about patient status and treatment plans.
  • Providing education and support to patients and their families to promote healthy behaviors and self-management skills.
  • Coordinating follow-up care and arranging appointments with specialists or other healthcare providers as needed.


Required Skills and Qualifications

To be successful in this role, you will need:



  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • At least two years of recent related clinical nursing experience in a community setting working with diverse populations, including those with mental health and substance use issues.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare teams.
  • Demonstrated ability to work independently and as part of a team, with a focus on patient-centered care and collaborative practice.
