At Lakeridge Health, we are seeking a highly skilled Patient Care Manager to lead our Healthy Aging Program. As a key member of our leadership team, you will be responsible for managing clinical service areas and providing strategic direction to interdisciplinary teams.

The successful candidate will have a strong background in healthcare leadership, with experience in quality improvement, project management, and change management. You will be expected to create a vision for positive change and innovation, inspiring and motivating teams to achieve excellence in patient care.

Key Responsibilities:

  • Lead and manage clinical service areas to ensure optimal performance and high-quality patient care
  • Provide strategic direction to interdisciplinary teams, facilitating collaboration and effective communication
  • Develop and implement evidence-based practice to achieve excellence in patient care
  • Contribute to the implementation of continuous improvement initiatives, utilizing skills and attributes associated with process improvement, project management, and change management

Requirements:

  • Bachelor's degree in a health-related discipline; Master's degree or current enrollment preferred
  • Current registration from the College of Nurses of Ontario
  • Minimum two years' recent clinical/leadership experience
  • Experience in quality improvement development, implementation, and evaluation
  • Project Management skills and experience
  • Demonstrated knowledge in related clinical specialty
  • Excellent verbal and written communication skills
  • Strong analytical skills, including data collection, analysis, sharing, and interpretation

Salary: $83,000 - $110,000 per year, depending on experience
