Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Palliative Outreach Resource Team at Island Health. As a key member of our team, you will provide expert support and care to individuals and families navigating palliative care.

Key Responsibilities
  • Provide comprehensive social and psychosocial assessments to identify client needs and develop effective care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and referrals to community resources.
  • Develop and maintain strong relationships with community agencies and healthcare providers to enhance access to services.
  • Provide counseling and support to clients and families, including crisis, loss, and grief counseling.
  • Conduct home visits and provide outreach services to clients in their communities.
Requirements
  • Bachelor of Social Work degree from a recognized university.
  • Two years of recent experience in palliative care, working with vulnerable populations.
  • Current full registration with the BC College of Social Workers.
  • Valid BC Driver's license.
Preferred Qualifications
  • Master of Social Work degree or equivalent.
  • Experience working with Indigenous communities and populations.
  • Knowledge of harm reduction and trauma-informed practice.
