Job Summary

Northern Health Authority is seeking a skilled Registered Nurse to join our team in Primary Care. As a Primary Care Nurse, you will provide high-quality patient care and work collaboratively with our interprofessional team to deliver exceptional healthcare services.

Key Responsibilities

• Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
• Work collaboratively with the interprofessional team to provide comprehensive care to individuals, groups, and communities.
• Develop and implement formal education programs for patients, families, and the general public.

Requirements

• Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
• Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
• Current valid BC Driver's License.
