About the Position:

As a Licensed Practical Nurse (LPN) with Interior Health, you will play a key role in planning, implementing, and evaluating patient access and care transition activities between IH facilities and healthcare services across the continuum of care.

You will work collaboratively with multidisciplinary and multi-agency teams to ensure seamless patient care, utilizing your clinical expertise to assess patient needs and facilitate access to appropriate services and resources.
