Job Title: Rehabilitation Service Specialist

Job Summary:

Lifemark is seeking a highly skilled Rehabilitation Service Specialist to join our team. As a key member of our rehabilitation services team, you will work closely with veterans and their families to provide comprehensive rehabilitation services and support.

Key Responsibilities:

  • Develop and implement individualized rehabilitation plans to support veterans in achieving their optimal wellness and independence.
  • Conduct assessments and provide recommendations to support a comprehensive rehabilitation plan.
  • Collaborate with medical, psycho-social, and vocational rehabilitation professionals to ensure a holistic approach to rehabilitation services.
  • Empower veterans to problem-solve and explore options for community-based interventions and resources to achieve desired outcomes.
  • Identify barriers or risk factors contributing to suboptimal outcomes and proactively develop and implement strategies.

Requirements:

  • Bachelor's degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field.
  • Minimum of one year experience in the direct provision of medical, psycho-social, or vocational rehabilitation services.
  • Experience working with persons living with mental or physical health problems creating barriers in life.
  • Employment conditional on obtaining federal security clearance – Reliability Status.
  • A reliable vehicle and the ability to travel.

Preferred Criteria:

  • Bilingual (French/English).
  • Experience working with employers, community resources, and training institutions.
  • Preference given to qualified former CAF members.
