Clinical Nurse Specialist

The Fraser Health Authority seeks a dedicated and compassionate Registered Nurse to join its team in a Transition Line role. This rotational position offers qualified registered nurses the opportunity to refine their skills by rotating through various specialty nursing units before or during post-graduate coursework relevant to their chosen specialty.
In this role, the Registered Nurse will provide direct patient care, assessing, planning, providing, and evaluating care in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health Authority. The nurse will collaborate with an interprofessional care team to provide person-centered care and participate in patient care and discharge planning processes upon admission.
The responsibilities of this position include:
- Conducting patient assessments and collecting information from diverse sources
- Collaborating with patients and their families/caregivers in developing care plans
- Participating in care and discharge planning processes for patients upon admission
- Coordinating patient nursing care
- Collaborating with members of the interprofessional care team
- Teaching patients and families/caregivers about illness prevention, health restoration, and health maintenance
- Discussing referral information with patients and their families/caregivers
- Documenting assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals
- Participating in nursing committees, care conferences, and councils
- Participating in research and quality improvement activities
- Attending in-services and other educational programs
- Providing feedback to the Manager/Delegate
- Performing other related duties as required
This role requires:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Completion of applicable post-graduate specialty nursing courses/programs or willingness to complete them
- Current certification in Basic Cardiac Life Support (BCLS)
- Ability to conduct initial and ongoing patient assessments and provide nursing care
- Ability to communicate effectively with patients, families/caregivers, and members of the interprofessional care team
- Ability to promote person-centered care
- Ability to integrate and evaluate data from multiple sources
- Ability to teach and instruct patients and families/caregivers
- Ability to organize and prioritize work
- Ability to collaborate with and promote cooperation among members of the interprofessional care team
- Ability to use factual information and basic principles and procedures to support nursing decisions and actions
- Ability to operate related equipment
- Physical ability to perform the duties of the position
