We are seeking a skilled and compassionate Nurse Care Coordinator to join our team in North Bay, ON. As a key member of our community nursing services team, you will play a vital role in developing and implementing effective care plans for our patients.
Key Responsibilities- Develop and maintain comprehensive care plans in collaboration with patients, family members, and healthcare professionals
- Provide exceptional nursing services to patients in their homes, clinics, and other community settings
- Communicate effectively with patients, families, and interdisciplinary care teams to ensure seamless care coordination
- Collaborate with healthcare professionals to identify and address patient needs, and develop strategies to improve patient outcomes
- Participate in quality improvement initiatives to enhance patient care and services
- Current registration with the College of Nurses of Ontario
- Valid driver's license and reliable vehicle for travel
- Excellent English verbal and written communication skills
- Current immunization record, including 2-step TB test
