Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Island Community Mental Health. As a LPN, you will play a vital role in providing high-quality care to our clients, promoting their recovery and well-being.

Job Summary:

The Licensed Practical Nurse will work in a licensed care mental health setting, performing duties within the competency guidelines and standards of practice outlined by the College of LPN's of BC and the current legislative scope of practice. The successful candidate will be responsible for assessing, planning, implementing, and evaluating nursing care to meet the needs of clients, as well as participating in an interdisciplinary team to develop, implement, and evaluate client care plans.

Key Responsibilities:
  • Assess and plan nursing care for clients, implementing and evaluating care to meet their needs.
  • Participate in an interdisciplinary team to develop, implement, and evaluate client care plans.
  • Administer medication to residents and provide medication reminders, in accordance with established policy and procedures.
  • Observe clients and their environments, ensuring pertinent information and significant clinical changes are communicated in a timely manner to the Director of Care, Manager, and team.
  • Monitor safety and report risks, near misses, incidents, accidents, and unsafe and/or hazardous conditions to Leadership.
  • Provide support and feedback to clients and their families.
  • Complete and maintain all documentation, including progress notes, observations, incident reporting, nursing interventions, and client charting, according to established standards, regulations, policies, and procedures.
Requirements:
  • Graduation from a recognized program for Practical Nurses.
  • Current full practicing license with the College of Licensed Practical Nurses of BC (CLPNBC) in good standing.
  • Certificates in CPR and First Aid, preferred.
  • Class 5 Driver's License, ASIST, Violence Prevention, Motivational Interviewing, and Harm Reduction an asset.
