About the Role

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at Pacific Coast Community Resources. As a Community Support Specialist, you will play a vital role in providing care and support to individuals living in staffed homes within their community.

Key Responsibilities
  1. Respect and Advocate: Ensure the supported persons' rights and choices are respected and advocate as needed.
  2. Effective Interactions: Interact effectively with people we support, fostering positive relationships and promoting independence.
  3. Empowerment and Community Integration: Empower the people we support to be full citizens of their community, encouraging participation in community activities and promoting social inclusion.
  4. Personal Service Plans: Participate in the development, implementation, and documentation of Personal Service Plans, ensuring individualized support and care.
  5. Communication and Collaboration: Foster positive relationships with persons' family members, professionals, co-workers, and members of the community, ensuring effective communication and collaboration.
  6. Record Keeping and Data Management: Record the daily activity and personal health information (PHI) of the people we support on our case management database, maintaining accurate and up-to-date records.
  7. Home Maintenance and Support: Work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties.
Requirements
  1. Education and Training: Community Support Worker Diploma or equivalent.
  2. Experience: Minimum 2 years of experience in community social services/health care.
  3. Certifications: Current Emergency First Aid/CPR and Food Safe certificates.
  4. Behavior Management: Previous behaviour management course/training.
  5. Communication Skills: Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
  6. Driver's License: Restricted class 4 driver's licence (for Full-Time positions).
  7. Criminal Record Clearance: Criminal record clearance for children and vulnerable adults.
  8. COVID-19 Vaccination: Proof of COVID-19 vaccination status.
  9. TB Test: TB Test.
  10. Computer Literacy: Moderate computer literacy.
