Community Support Specialist

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at Pacific Coast Community Resources. As a Community Support Specialist, you will play a vital role in providing care and support to individuals living in staffed homes within their community.
Key Responsibilities- Respect and Advocate: Ensure the supported persons' rights and choices are respected and advocate as needed.
- Effective Interactions: Interact effectively with people we support, fostering positive relationships and promoting independence.
- Empowerment and Community Integration: Empower the people we support to be full citizens of their community, encouraging participation in community activities and promoting social inclusion.
- Personal Service Plans: Participate in the development, implementation, and documentation of Personal Service Plans, ensuring individualized support and care.
- Communication and Collaboration: Foster positive relationships with persons' family members, professionals, co-workers, and members of the community, ensuring effective communication and collaboration.
- Record Keeping and Data Management: Record the daily activity and personal health information (PHI) of the people we support on our case management database, maintaining accurate and up-to-date records.
- Home Maintenance and Support: Work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties.
- Education and Training: Community Support Worker Diploma or equivalent.
- Experience: Minimum 2 years of experience in community social services/health care.
- Certifications: Current Emergency First Aid/CPR and Food Safe certificates.
- Behavior Management: Previous behaviour management course/training.
- Communication Skills: Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
- Driver's License: Restricted class 4 driver's licence (for Full-Time positions).
- Criminal Record Clearance: Criminal record clearance for children and vulnerable adults.
- COVID-19 Vaccination: Proof of COVID-19 vaccination status.
- TB Test: TB Test.
- Computer Literacy: Moderate computer literacy.
