Vancouver Coastal Health is seeking a skilled Social Worker to join our Long Term Care Team at Powell River Health Unit in Powell River, BC.
Key Responsibilities- Work in partnership with clients and families to establish goals and develop care plans.
- Assess service eligibility and facilitate the development and implementation of care plans.
- Deliver direct care and provide care management to assist clients in managing their own care.
- Work in consultation with other system partners and incorporate current evidence into practice.

Qualifications- Master's degree in Social Work from an accredited University.
- Current full registration with the British Columbia College of Social Workers (BCCSW).
- Recent, related experience in Home Health clinical practice in Community.
- Valid BC Driver's License required.
