Job Title: Nurse Care Manager

Mon Sheong Foundation is seeking a skilled and compassionate Nurse Care Manager to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality care to our residents and ensuring their needs are met.

Key Responsibilities:
  • Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery and effective communication.
  • Provide education and support to residents and their families on health-related topics and care management.
  • Conduct regular assessments and evaluations to identify areas for improvement and implement changes as needed.
  • Manage and coordinate care services, including medication management, wound care, and other specialized services.
  • Develop and maintain relationships with community resources and healthcare providers to ensure residents receive comprehensive care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Current certificate of competence as a Registered Nurse from the College of Nurses of Ontario.
  • Valid CPR (Health Care Provider Level C/Basic Life Support) and First Aid certificates.
  • Minimum of one year experience in geriatric, rehabilitation, or long-term care.
  • Ability to work shift duties (Day/Evening/Night) and weekends at a rotation basis.
  • Strong assessment and communication skills, with the ability to work effectively in a team environment.
  • Proficiency in oral and written English, with the ability to communicate with residents and families from diverse backgrounds.
Preferred Qualifications:
  • Certification in Gerontology and Current membership with GNA and RNAO.
  • Relevant computer literacy and experience with electronic medical records.
