About the Role

We are seeking a skilled and compassionate Community Health Specialist to join our team at CarePartners. As a key member of our interdisciplinary care team, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and maintain effective care plans in collaboration with patients, family members, and other healthcare professionals.
  • Provide high-quality nursing services, including assessments, interventions, and education, to patients in various settings.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of best practices in community health care.

Requirements
  • Current registration with the College of Nurses of Ontario (CNO) in good standing.
  • A strong commitment to providing exceptional patient care and a passion for community health.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • A valid driver's license and reliable vehicle, as regular travel is required.
