About the Role

CarePartners is a leading provider of home health care services, seeking a skilled and compassionate Visiting Nurse to join our team. As a Visiting Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Develop and implement effective care plans in collaboration with the Home and Community Care Support Services (HCCSS)
  • Provide exceptional nursing services to patients in their homes, including one-on-one care and virtual consultations
  • Communicate and liaise with patients, family members, and the interdisciplinary care team to ensure seamless care coordination
  • Perform other clinical duties as required, including administering medications and providing wound care
Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills and ability to work independently
  • Valid driver's license and vehicle, with regular travel required
  • Excellent English verbal and written communication skills
  • CPR Certification and current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
