About the Role

CarePartners is seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will have the opportunity to make a meaningful difference in the lives of our patients by providing exceptional nursing services in a variety of settings.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide nursing services to patients in their homes, clinics, retirement homes, or schools
  • Communicate and collaborate with patients, family members, and other healthcare professionals to ensure seamless care
  • Perform other clinical duties as required
Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
