Job Title: Registered Nurse - Rapid Response

Job Summary:

The Registered Nurse - Rapid Response is a critical role in providing high-quality care to patients with complex/chronic diseases. As a member of our multi-disciplinary team, you will be responsible for delivering exceptional patient care, ensuring seamless transitions from acute care to home care, and promoting health coaching and disease-specific education.

Key Responsibilities:

  • Provide in-home nursing visits within 24 hours of hospital discharge for patients with complex/chronic diseases.
  • Conduct clinical, problem-based assessments to identify patient needs and develop personalized care plans.
  • Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care.
  • Perform medication reconciliation and review medication protocols with patients and caregivers.
  • Utilize a "teach back" approach to educate patients and families on care plans, treatments, and symptom management.
  • Initiate communication with primary care providers and ensure follow-up within 7 days.
  • Refer patients to longer-term community programs as needed, once goals of the Rapid Response program are met.

Requirements:

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum 3-5 years clinical experience working as a Registered Nurse.
  • Recent and relevant experience providing direct nursing care within areas of Paediatrics, Chronic Disease Management, Medicine, or Cardiology.
  • Superior clinical assessment skills and ability to provide quality clinical service delivery.
  • Ability to work independently and effectively communicate with patients, families, and healthcare providers.
  • Strong critical thinking and problem-solving skills.
  • Advanced oral and written proficiency in English is essential.
  • Valid driver's license and access to a vehicle, as travel to patient homes will be required.
  • Valid Emergency First Aid Certification.
