Position Overview

We are seeking a compassionate and skilled Social Services Specialist to join our dedicated team within the Sioux Lookout Diabetes Program. This role will involve collaborating closely with patients, families, and healthcare professionals to provide holistic support and resources.

Responsibilities
  • Provide case management services to adults and children living with diabetes, focusing on addressing socioeconomic factors that impact health outcomes.
  • Conduct thorough assessments of patient needs and develop individualized care plans in collaboration with patients and their families.
  • Work collaboratively with the interdisciplinary diabetes team and broader circle of care to ensure comprehensive patient support.
  • Connect patients with community resources and support services, including assistance with social assistance applications and referrals to counselling or therapy.
  • Assist children and families in developing healthy coping mechanisms and navigating the challenges associated with diabetes.
  • Provide crisis intervention and guidance as needed, ensuring patient safety and well-being.
  • Maintain accurate patient records and documentation in accordance with established protocols.
Qualifications
  • Post-secondary education in Social Service Work, Social Work, or a related field.
  • Registration with the Ontario College of Social Workers and Social Service Workers, or eligibility for registration.
  • Proven ability to analyze client needs and develop effective solutions.
  • Excellent communication, interpersonal, and conflict resolution skills.
  • Experience working in a healthcare setting is an asset.
  • Training or experience in cognitive behavioral therapy, crisis intervention, motivational interviewing, or other relevant modalities is highly desirable.
