Job Summary

Vancouver Coastal Health is seeking a skilled Licensed Practical Nurse (LPN) to fill the role of Home Support Supervisor at Central & West Community Health Centre in North Vancouver, BC.

Key Responsibilities
  • Assess and develop care plans for clients with acute, chronic, and palliative conditions, working collaboratively with healthcare teams and clients to establish realistic goals.
  • Deliver direct care and provide care management to assist clients in navigating community services, ensuring safe and effective care.
  • Supervise and coach team members to promote a high-performing team environment, fostering open communication and collaboration.
Qualifications
  • Graduate of a recognized LPN program.
  • Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years' experience working with geriatric clients, including one year in a supervisory role.
  • Valid BC Class 5 Driver's License.
