Intensive Case Manager

We are seeking a highly skilled and compassionate Intensive Case Manager to join our team at Island Health. As a key member of our community support team, you will work closely with clients who are struggling with substance use and mental health issues.
Key Responsibilities- Provide intensive case management services to clients with complex needs, including substance use and mental health concerns.
- Develop and implement individualized treatment plans that address the client's unique needs and goals.
- Work collaboratively with an interdisciplinary team to provide comprehensive care and support to clients.
- Provide education and support to clients and their families on harm reduction strategies and mental health management.
- Conduct regular assessments and evaluations to monitor client progress and adjust treatment plans as needed.
- Develop and maintain relationships with community resources and services to ensure clients have access to necessary support.
- Baccalaureate degree in a health-related discipline from an approved post-secondary institution.
- Recognized post-basic education related to mental illness and/or substance use.
- Two years of recent related experience supporting clients with substance use and mental health issues in a community setting.
- Strong knowledge of harm reduction philosophy and practice, as well as HIV, HCV, and related issues.
- Valid BC Driver's License.
- Demonstrated competence in the treatment of addiction and mental illness congruent with Island Health's framework and standards.
- Ability to practice from a harm reduction approach.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of an interdisciplinary team.
- Strong organizational and problem-solving skills.
