Job Title: Community Mental Health Nurse

Health Careers in Saskatchewan is seeking a skilled Community Mental Health Nurse to join our multidisciplinary team. As a Community Mental Health Nurse, you will play a vital role in providing intensive case management to individuals with chronic and persistent mental illness.

Key Responsibilities:
  • Provide intensive case management to individuals with chronic and persistent mental illness
  • Develop and implement individualized treatment plans
  • Collaborate with multidisciplinary teams to provide comprehensive care
  • Conduct assessments and evaluations to identify client needs
  • Develop and maintain relationships with community agencies and organizations
Requirements:
  • Minimum 5 years of experience working with clients with severe and persistent mental illness
  • Minimum 5 years of recent experience in a community or acute mental health setting
  • Bachelor of Science in Nursing (BScN) degree
  • Valid Class 5 driver's license
  • Registration with the College of Registered Nurses of Saskatchewan (CRNS)
