Job Summary

The EMP is seeking a dedicated and compassionate Nurse Coordinator to join our Special Care Home (SCH) unit. As a key member of our team, you will play a vital role in providing exceptional care to our residents by conducting thorough assessments, coordinating care, and collaborating with healthcare professionals.

Key Responsibilities:

  • Conduct comprehensive assessments of SCH residents, review medical charts, and reassess client needs regularly to ensure optimal care.
  • Provide oversight for updating patient care plans and educate staff on individual care needs to ensure seamless care delivery.
  • Collaborate with SCH, Social Development, and other organizations as necessary to ensure comprehensive care and support.
  • Deliver education to residents, their caregivers, and SCH staff on various topics to promote health and well-being.
  • Coordinate vaccination campaigns within the SCH and administer vaccinations to protect our residents.
  • Offer clinical support during outbreaks while maintaining high-quality care for residents.

Requirements and Qualifications:

  • Nursing Diploma or Bachelor's Degree in Nursing.
  • Registration with the Nurses Association of New Brunswick.
  • Minimum of two years of specialized nursing experience within the last five years.
  • Knowledge of Extra-Mural Services and community resources.
  • Possession of a valid New Brunswick Class 5 Driver's License.
  • Clear criminal record and current CPR Level C certification.
  • Strong teamwork, leadership, and interpersonal skills.
  • Flexibility to work all assigned shifts.
  • Respect for confidentiality and professional work ethic.
