Job Title: Registered Nurse - Rapid Response

Job Summary:

The Registered Nurse - Rapid Response provides high-quality care to patients with complex/chronic diseases during their transition from acute care to home care. This role focuses on delivering timely and effective nursing interventions to ensure a smooth transition and optimal patient outcomes.

Key Responsibilities:

  • Conduct comprehensive assessments of patients with complex/chronic diseases, including CHF, COPD, and Diabetes.
  • Develop and implement individualized care plans to address patient needs and goals.
  • Provide education and support to patients and their caregivers on disease management, symptom control, and medication adherence.
  • Collaborate with the patient's primary care provider and other healthcare professionals to ensure seamless care coordination.
  • Refer patients to community programs and resources as needed to support their ongoing care.

Requirements:

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum 3-5 years of clinical experience in a relevant area, such as Paediatrics, Chronic Disease Management, Medicine, or Cardiology.
  • Recent and relevant experience providing direct nursing care to patients with complex/chronic diseases.
  • Superior clinical assessment and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong communication and interpersonal skills.
