Position Overview:

This role is for a Personal Support Worker who will deliver essential personal assistance and household support to clients and their families, adhering to the policies and standards set forth by the organization. The PSW will work collaboratively with healthcare professionals to ensure clients achieve the highest possible level of independence and well-being.

Employment Details:

  • Status: Permanent, Part-Time (0.4 FTE)
  • Program: Assisted Living Services for High-Risk Seniors
  • Hours: 30 hours bi-weekly
  • Schedule: Days, Evenings, Weekends
  • Union: OPSEU
  • On-Call: Yes

Key Responsibilities:

  1. Assess and evaluate the living conditions and care needs of clients and families, making recommendations for adjustments to the care plan as necessary.
  2. Assist in developing personalized care plans under the guidance of the Personal Support Program Manager, ensuring all relevant information is accurately documented.
  3. Implement necessary modifications to care plans to ensure safety and compliance.
  4. Communicate any changes in client needs or home circumstances to the appropriate supervisor.
  5. Perform various personal support tasks tailored to the needs of clients, including light housekeeping, meal preparation, laundry, shopping, and ensuring a safe living environment.
  6. Provide personal care assistance as directed, including mobility support, hygiene assistance, and other daily living activities.
  7. Deliver basic bedside care under the supervision of nursing staff when required.
  8. Offer respite care services to clients and support their families by providing childcare as needed.
  9. Organize daily activities and prioritize tasks effectively under the direction of the Personal Support Program Manager.
  10. Complete necessary documentation and reports to maintain operational efficiency.
  11. Evaluate and understand clients' communication patterns and their ability to engage.
  12. Collaborate with healthcare team members to address the physical and emotional needs of clients and inform supervisors of any concerns.
  13. Adhere to occupational health and safety regulations and organizational policies.
  14. Follow risk management protocols to ensure client safety.
  15. Perform additional duties as assigned.

Relationships:

Maintain regular communication with supervisors and the Personal Support Program Manager regarding client care plans and any pertinent information.

Qualifications:

  • High school diploma required; secondary education preferred.
  • Certification as a Continuing Care Assistant (CCA) is necessary in Nova Scotia.
  • Completion of a Personal Support Worker program or equivalent Health Care Aide training.
  • Current Standard First Aid and CPR level C certification is required.
  • Experience in household management and working with elderly, ill individuals, or children is preferred.

  • Strong assessment and organizational skills.
  • Knowledge of community resources.
  • Capability to work both independently and as part of a multidisciplinary team.
  • Willingness to follow directions and adapt to various environments.
  • Commitment to maintaining confidentiality.

Additional Requirements:

  • Access to a personal vehicle for work-related duties, along with a valid driver's license and proof of insurance.
  • A satisfactory Criminal Records Check is mandatory.
  • A Vulnerable Sector Search and/or Child Abuse Registry Check may be necessary.
  • Flexibility in working hours is essential.
  • Ability to wear Personal Protective Equipment (PPE) as required.

Work Environment: This position requires attention to detail, physical stamina for lifting and carrying, and the ability to perform various physical tasks safely.
