About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Rapid Response Nurse. In this critical role, you will be responsible for ensuring seamless transitions from hospital to home for our patients, particularly those with complex needs and high-risk characteristics.

Key Responsibilities
  • Provide timely and effective care to patients in their homes, focusing on frail adults and seniors with complex needs.
  • Collaborate with primary care teams, hospitals, and community resources to ensure a smooth transition of care.
  • Assess patient needs, develop care plans, and coordinate services to promote optimal health outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
  • Participate in quality improvement initiatives and contribute to the development of best practices in care transition.
Requirements
  • University degree in Registered Nursing.
  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum of 2 years of relevant experience in geriatrics or pediatrics.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in computerized information systems.
