Job Title: Social Worker

We are seeking a skilled Social Worker to join our interdisciplinary team at the New Vision Family Health Team. As a key member of our team, you will play a vital role in providing comprehensive care to our patients.

Job Summary:

The successful candidate will possess a Master of Social Work (MSW) degree and registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW). A minimum of 3 years of relevant experience in areas such as intake, crisis intervention, creating safety plans, triage, system navigation, and individual counseling is required.

Key Responsibilities:
  • Provide individual counseling support to patients.
  • Assess and navigate patients referred by primary care physicians or other healthcare team members.
  • Facilitate initial screening and provide resources as needed.
  • Provide referral information for community resources and services.
  • Assist patients and their families in navigating the healthcare and social services system.
  • Facilitate groups in response to specific needs or populations identified by the team.
  • Work with community groups and organizations to coordinate care.
  • Advocate for patients who encounter difficulties accessing health education programs and services.
Qualifications:
  • Master of Social Work (MSW) degree and registration in good standing with OCSWSSW.
  • Minimum 3 years of relevant experience.
  • Case management and system navigation skills.
  • Trauma-informed practice.
  • Excellent clinical skills and experience with assessment, treatment, and management of mental health problems.
Requirements:
  • Availability to work at least one evening per week.
  • Valid professional liability insurance.
  • Vulnerable Sector Check required.
Compensation and Benefits:
  • Hourly rate up to $38.54 ($75,153 annually).
  • Healthcare of Ontario Pension Plan (HOOPP).
  • Extended healthcare plan.
  • 5 weeks paid vacation per year.
  • 8 paid sick/personal days per year.
  • Paid day on birthday each year (following one year of employment).
