Registered Practical Nurse

VON Canada is seeking a skilled Registered Practical Nurse to join our team as a Home Care Specialist. In this role, you will provide high-quality, client-centered care to individuals and families in their homes and communities.
Key Responsibilities- Provide nursing care to clients with predictable care needs, utilizing established plans of care.
- Manage and facilitate care through established nursing care plans, evaluating client responses to interventions.
- Provide cost-effective, organized, and outcome-directed care on an ongoing basis.
- Conduct comprehensive assessments of clients and their families, utilizing established procedures.
- Document all relevant information clearly, concisely, and accurately, adhering to VON standards.
- Collaborate with healthcare teams to develop and implement care plans, selecting and implementing strategies or interventions as needed.
- Negotiate with clients and families during care delivery, respecting their choices and preferences.
- Stay up-to-date with current research findings and incorporate them into clinical practices.
- Refer clients to appropriate nursing care providers and other disciplines, utilizing established guidelines.
- Promote client-directed care, self-care, and independence through teaching and education.
- Licensure as a Registered or Licensed Practical Nurse in the province of practice.
- Clinical practice experience is an asset.
- Experience in community health is an asset.
- Strong written and verbal communication skills, with particular proficiency in interviewing relevant stakeholders.
- Ability to observe and make effective assessments of clinical issues.
- Capacity to adhere to strict confidentiality.
- Effective time management skills that allow for independent planning and organizing of daily work activity.
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
