About Our Opportunity

We are seeking a highly skilled and compassionate Care Coordinator to join our team in the Disability Support Program. As a Care Coordinator, you will play a vital role in supporting individuals with disabilities, providing them with the necessary care and supervision to achieve their goals and improve their quality of life.

Primary Accountabilities

Key responsibilities of this role include:

  • Assessing individuals with disabilities to determine their level of support and developing personalized care plans
  • Coordinating and implementing programs and services to meet the assessed needs of individuals with disabilities
  • Working in partnership with individuals with disabilities, their families, and other stakeholders to provide support and guidance
  • Collaborating with other departments and agencies to provide a continuum of services and support
Qualifications and Experience

To be successful in this role, you will require:

  • A degree in a relevant field, such as nursing, social work, occupational therapy, or physiotherapy
  • At least five years of experience in a related field, preferably in a care coordination or support role
  • Excellent assessment, interviewing, and negotiation skills
  • Strong critical thinking, problem-solving, and time management skills
  • A solid knowledge base in child development and a thorough understanding of the needs and issues relating to children, adults, and families with disabilities
