We are seeking a highly skilled and motivated Registered Dietitian to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will work collaboratively to provide high-quality patient care and support the development of a person and family-centered model of care.
Key Responsibilities- Develop and implement individualized nutrition plans for patients with various health conditions, including diabetes, kidney disease, and chronic diseases.
- Provide education and counseling to patients and their families on healthy eating habits, nutrition, and lifestyle modifications.
- Collaborate with healthcare providers to coordinate care and ensure seamless transitions between healthcare settings.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to nutrition and dietetics.
- Stay up-to-date with the latest research and evidence-based practices in nutrition and dietetics, and apply this knowledge to improve patient outcomes.
- Baccalaureate degree in nutrition and dietetics.
- Successful completion of an accredited dietetic internship program (or equivalent acceptable to the facility).
- Registrant of the College of Dietitians of British Columbia.
- Two years of clinical nutrition experience in a healthcare environment or an equivalent combination of education, training, or experience.
- Practice-specific requirements and experience in programs such as diabetes education, nutrition support, and kidney and chronic disease management programs.
- Valid BC Driver's License.
