Community Health Nurse

We are seeking a skilled and compassionate Community Health Nurse to join our team at Fraser Health Authority. As a Community Health Nurse, you will work independently in the community setting, collaborating with an interprofessional team to provide high-quality care to clients and their families.
Key Responsibilities- Establish therapeutic relationships with clients and their families, ensuring their autonomy and dignity in decision-making and care planning.
- Conduct initial and ongoing client assessments, providing individualized care and interventions, and referring clients to other services as needed.
- Develop comprehensive care plans in collaboration with the interprofessional team, primary care providers, and clients/families.
- Provide direct client care, including health counseling, education, and self-management support, using care management principles.
- Collaborate with other healthcare professionals to facilitate the flow of information and coordinate care services.
- Maintain accurate clinical records and statistical information, adhering to established policies and procedures.
- Participate in quality improvement and risk management activities, identifying client care issues and collecting data.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
