Job Title: Support Worker, Service Coordination

Community Living Mississauga is seeking a dedicated and compassionate Support Worker to join our team in providing individualized service coordination support for individuals with intellectual disabilities and their families.

Job Summary:

This role involves working under the guidance of a Supervisor or Manager to provide service coordination support based on goals identified by individuals and their families. The successful candidate will be responsible for making appropriate referrals to community services and programs, promoting community inclusion, independence, and self-determination, and working collaboratively with colleagues, community partners, and families to solve problems and resolve conflicts.

Key Responsibilities:
  • Provide service coordination support based on goals identified by individuals and their families.
  • Make appropriate referrals to community services and programs to address identified goals.
  • Promote community inclusion, independence, and self-determination for individuals supported.
  • Work collaboratively with colleagues, community partners, and families to solve problems and resolve conflicts.
  • Identify and report support situations that do not meet organizational standards.
  • Utilize and develop new techniques, options, or approaches to supporting individuals and their families.
Qualifications:

Developmental Services Worker Diploma (preferred) or Social Services Worker Diploma, Child and Youth Worker Diploma with a minimum of 1 year of experience working or volunteering with individuals with intellectual disabilities. Will also consider a University Degree with a minimum of 1 year of experience working or volunteering with individuals with intellectual disabilities. Must have a valid Ontario Driver's License (Class G or G2), access to a reliable vehicle, and insurance. Current valid First Aid/CPR certificate required. Proficient with PC computer environment; MS Office (Word, Excel, Outlook, etc.) and other software. Excellent interpersonal and communication skills (verbal, non-verbal, written). Ability to work collaboratively in a team environment and with families as necessary. Good problem-solving and conflict resolution skills. Bilingualism in English/French or other language would be considered an asset.
