Job Title: Mental Health Program Manager

Sienna Senior Living is seeking an exceptional individual to fill a key management position, a role that is pivotal in bringing our core values of positivity, accountability, community, and caring to life.

Job Summary:

The Manager, Mental Health Programs plays a crucial role in supporting the development, implementation, and sustainability of care pathways focused on serving residents with mental health needs in the long-term care community.

Key Responsibilities:
  • Develop specific criteria and attributes that would identify residents with mental health and clinical needs
  • Research, design, and implement all components of mental health programs
  • Provide consultation, assessment, development, implementation, and evaluation of individual plans of care
  • Create a measurement-based program plan
  • Design an approach for urgent and developing mental health needs
  • Conduct rounds to ensure plans of care are meeting the needs of the residents
  • Collaborate with leadership and team members to ensure programs are designed and implemented aligns with Sienna mission, vision, and values
Requirements:
  • You are a Registered Nurse, an Advanced Practice Nursing, or a Regulated Health Professional in a relevant field. You are in good standing with your associated regulatory College
  • Require 5 years clinical experience involving clients with mental health and addiction issues
  • Design experience in developing programs using evidence-based practices
  • Experience in leading change, monitoring, and measuring outcomes, and the ability to provide advanced clinical care
  • You have excellent interpersonal, communication, negotiation, teaching, and computer skills
