Nurse Case Manager

Vancouver Coastal Health is seeking a Registered Psychiatric Nurse or Registered Nurse to join our Long Term Care team in Sechelt, BC.
Key Responsibilities- Manage a caseload of clients with chronic illnesses or long-term physical disabilities.
- Conduct assessments, identify problems, and develop plans of care in collaboration with interdisciplinary teams.
- Provide continuity of care across the continuum by liaising with healthcare professionals and coordinating care.
- Develop and maintain formal partnerships with General Practitioners.
- Act as a clinical resource for the interdisciplinary team, providing information on nursing theory and practice.

Qualifications- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years' recent clinical nursing experience, preferably in acute care or case management.
- Valid BC Driver's License and local area travel.
